The Risk of Antibiotic Resistance in the Outpatient Setting

Antibiotic development has declined in recent decades, limiting the options for treating increasingly resistant infections and compounding the problem of antibiotic resistance.

The inappropriate prescription of antimicrobials—including incorrect dosage or duration of an appropriate antimicrobial—contributes to antibiotic resistance. Antibiotics are commonly prescribed when they are not necessary2 for a range of conditions, including:

  • Viral infections
  • Noninfectious processes
  • Bacterial infections that do not require antibiotics (such as small skin abscesses that will resolve with incision and drainage)
  • Bacterial colonization (such as a positive urine culture result in a patient with a bladder catheter)

The CDC identifies the following risks3 from the misuse of antibiotics:

  • Antibiotics can cause adverse drug events and promote antibiotic resistance.
  • There are more Clostridium difficile infections in places with more antibiotic use.
  • Antibiotic use in primary care is associated with antibiotic resistance at the individual patient level.
  • Antibiotics cause 1 in 5 emergency department visits for adverse drug events.

Antimicrobial Stewardship in the Pediatric Setting

Patients are at risk for antibiotic-resistant infection regardless of their demographics or where they are treated, but the pediatric population is particularly at risk because of:

  • Different antimicrobial needs and uses
  • Unique dosing considerations
  • Vulnerability for resistance due to a lifetime of antibiotic exposure
  • An increased risk of adverse events4

Antibiotics are the most common cause of emergency department visits for adverse drug events in children.5

The main goal of an ASP is to ensure each patient receives the most appropriate antimicrobial with the correct dose and duration. Proper ASPs can lead to:
